What sits beneath

The sternum (breastbone) is a flat, vertically-oriented bone running down the center of the chest, between and slightly beneath the pectoral muscles and breasts. It consists of three parts: manubrium (top), body (middle), and xiphoid process (bottom tip). Skin over the sternum is thin (1.2–1.5mm) with minimal subcutaneous fat—the bone sits very close to the surface. There is essentially no cushioning between needle and hard bone. Intercostal nerves run on both sides of the sternum; branches extend directly over it. The sternum is highly innervated, meaning nerve density is very high. Women have the added complexity of breast tissue and supporting ligaments beneath the skin surface. The sternum experiences constant micro-motion from breathing and is under continuous tension from chest muscle contractions.

Sternum healing is extended and painful because of constant chest movement, breathing, and skin tension. The bone beneath offers no give; scabs are easily reopened by stretching or movement. Tight clothing (shirts, sports bras) and bra straps create pressure that delays healing. For women, bra straps apply direct pressure over the healing area, compressing it with every movement. Sleeping is uncomfortable in any position. Laughing, coughing, and deep breathing all irritate the wound. Sweat and friction from sleeping position are major factors.

Deep breathing should be normal, but avoid intentional breath-holding. Avoid swimming and soaking for at least the first week. Keep the area clean and moisturized, but avoid excessive touching. Any pressure or friction will reopen scabs and delay healing.
